They accept requests via email or mail. They charge .50 a page for whatever copies are made. Right now, they are back logged about 4-6 weeks. The good part is that they really research the information for you. With all the detail you have, I am sure it will be a successful. Of course, these records only go to 1915. The occurrences after 1915 will have to found through a different avenue. But this is a good first step to narrow the time frame. I've found that I tend to get a quicker response to mailed requests accompanied by a small amount of money to cover the costs upfront.
