    1. [MABERKSH] How to go about getting a death certificate from about 1918..
    2. donald petravick
    3. My grandfather, Simon Theodore Petravick died in May, 1918 and is likely buried in St Joseph's cemetary in North Adams. I'm not sure where his residence was. I am guessing N. Adams because of the place of burial, and because his borhter in law resided there. He sold a farm in Cheshire in 1915. He was married in St. Stan's in Adams in 1902. How does one find a death record for him? -- Don __________________________________________________ Do You Yahoo!? Tired of spam? Yahoo! 3. Hi Don, You can request his death record through MA Health and Human Services. Here is the direct link to that section of their webpage. http://www.mass.gov/?pageID=eohhs2terminal&L=4&L0=Home&L1=Consumer&L2=Basic+Needs&L3=Vital+Records&sid=Eeohhs2&b=terminalcontent&f=dph_vital_records_c_obtaining_copies&csid=Eeohhs2 It will give you all the information you will need to do your request. Good luck! Terri --- donald petravick <[email protected]> wrote: > > My grandfather, Simon Theodore Petravick died in > May, > 1918 and is likely buried in St Joseph's cemetary in > North Adams.
