My grandfather, Simon Theodore Petravick died in May, 1918 and is likely buried in St Joseph's cemetary in North Adams. I'm not sure where his residence was. I am guessing N. Adams because of the place of burial, and because his borhter in law resided there. He sold a farm in Cheshire in 1915. He was married in St. Stan's in Adams in 1902. How does one find a death record for him? -- Don __________________________________________________ Do You Yahoo!? Tired of spam? Yahoo! Mail has the best spam protection around http://mail.yahoo.com
Hi Don, You can request his death record through MA Health and Human Services. Here is the direct link to that section of their webpage. http://www.mass.gov/?pageID=eohhs2terminal&L=4&L0=Home&L1=Consumer&L2=Basic+Needs&L3=Vital+Records&sid=Eeohhs2&b=terminalcontent&f=dph_vital_records_c_obtaining_copies&csid=Eeohhs2 It will give you all the information you will need to do your request. Good luck! Terri --- donald petravick <[email protected]> wrote: > > My grandfather, Simon Theodore Petravick died in > May, > 1918 and is likely buried in St Joseph's cemetary in > North Adams.
