Kailynn Nicole Smith MANSFIELD: Kailynn Nicole Smith, 5 weeks, of Mansfield, passed away on April 30, 2015, at Nationwide Children's Hospital, Columbus. Kailynn was born prematurely on March 22, 2015, at Ohio State University, to Michael Smith and Jessica Jacobs of Mansfield. In her short life, she touched so many lives. She is survived by her parents; three brothers, Dacota Smith, Logan Loveless, and Landon Lewis; grandparents, Brian and Michelle Moore; great-grandpa, Paul Shade; aunt and uncle, AJ Moore and Cody Lamb; three uncles, Brandon Moore, Scott Shade, and Austin Bigley; and three close friends, Amanda McCracken, JR Burns, and Phillip Isaac Sr. Kailynn was preceded in death by special friend, Ashley Glaze; and great-grandpa, Pete Henry Smith. Calling hours will be from 10:00 a.m. to 12:00 p.m. on Wednesday, May 6, 2015, at Trinity United Methodist Church, 1592 Grace St. Funeral services will follow at 12:00 p.m. with Pastor Bob Stull officiating.
