    2. As a member of the DAR (Daughters of the American Revolution), I can tell you that applications approved BEFORE 1958 needed to be re-proved. This application sounds like it might have been made before then. Also, this sounds like a short form application. This doesn't give alot of information. If you want, you can send a check for $5, payable to Treasurer General, NSDAR and send it to: NSDAR Record Copy Department 1776 D Street NW Washington, DC 20006 When writing, ask for (a) the application of the patriot for the child you descend from or (b) the most recent application.
