Their is a Form page where you can print a copy of Standard Form 180. SF 180 has two sections. The front page is for the Veterans Information and the back page has the address to send it to depending on which service the Veteran was in. Make sure that you set your printer to Legal size and have 8 1/2" x 14" paper in it. Also if you want the Veteran's medical records, check the "Undeleted" part on the front page.
