I think the most likely explanation for these situations is one where there was no attending physician, and also the family took care of the burials themselves without a funeral home. Thus without either a doctor or funeral home to generate a death certificate, none was generated. As to obituaries, the Harlan Enterprise is extant on microfilm at the UK libraries for those years, and you can order the microfilms via interlibrary loan according to their newspaper website which can be found here: http://lib.uky.edu/repro/ So there likely are not any death certs to be found, and your last likely source is that newspaper.
