Russ: Birth certificates from 1911 on can be obtained from the Ky. Office of Vital Statistics for $9.00 each. The address is: Vital Statistics; 275 E. Main St., Frankfort, KY 40621. Birth certificates BEFORE 1911 can sometimes be obtained from the Archives Dept., but you have to submit the request on a particular form, which you can get by calling them at (502) 564-8300. When they send you the form, you send it in along with $12.00 if you do not live in Kentucky. If you DO live in Kentucky, they will bill you after they've done the search. If they don't have the document, they return the $, at least they did when they couldn't find what I requested recently.
