This sounds like it has a sad story behind it, and I'm sorry for that... Have you thought of ordering his death certificate? That would tell you who the informant was, the cemetery, the doctor, hospital, cause of death, etc. Also, you might contact the Leavenworth Times newspaper. It has his obituary in it on page A5, Sept 23, 1991 issue. I'm sure Directory Assistance could give you their telephone number, or you might be able to find it online. Good luck.
