One copy costs $15. The web address is: www.kdheks.gov/vital/death The info. I provided probably gives the same info. as it was from the clerk's report that turned in the information for creating the death certificate file at vital records. Parents names are not on the printed forms for death certificates in 1925, so unlikely to be there unless it was added later.
