USA Death certificate acquisition depends on what state it is. Mandatory Registration began at statehood which varies for almost every state. Original 13 colonies go back to late 1700s but not sure if each and every one of them began registration the same year. To confuse matters more, every state 's registrations are a bit different. Basics are there, but other questions vary widely if any others were asked at all. Most certificates did ask parent's names, but if they were not known, it was simply written 'unknown'. Another problem in the US is that death certificates can sometimes only be obtained by members of the immediate family, depends on the state again. One volunteer at the FHC here had applied for the Death registration of her mother which occured perhaps 5y before the written request. They returned her application and asked for the mother to sign it first. Uhhhh, the worker wrote back and explained that it should have been obvious why the mother could not sign her own death certificate request. Again the document came back with pointed notations showing where her mother needed to SIGN the request. This time the volunteer just SIGNED the request with her mother's name. She promptly got the certificate. I made a request for a 1933 death certificate in Nevada, had to state my relationship to the deceased so I wrote 'Niece', it worked and I was sent the paper. The LDS church maintains a booklet of forms for every state in the union to request a certificate by post. If you need a JPG of any state's request form, write to me and I'll scan one and send it.
