There follows: No.__ Date & Place of Birth ________ Name if any _____ Sex ____ Name & surname & Dwelling Place of Father___ Name & surname & Maiden Name of Mother __ Rank or Profession of Father ____ Signature, Qualification & Residence of Informant (usually the father)______ When registered._____ Signature of Registrar ______ Baptismal name if added after the notification of birth date ______Usually blank because people did not bother to go back and add it in. Death Certificates generally have in addition to the Header information similar to the birth certificate about the District etc. the following specifics.
