John: I've written for baptism cert. with a lot of success. Sometimes more info on these then NYC cert.! I usually know the name of the church and would give the rectory a call to get the name of the pastor. I would the write my letter/request to the pastor for the baptismal cert with a name, date range and if possible parents' names.
