Does anyone have a "form" that I could reprint to send to a relative to fill out for genealogy? I could make one up, but thought maybe someone already has one. It would include a place to put birth, death dates, marriage, children, place of birth, etc. Jane

You would need Adobe Reader to open them but you can print them out once you have the file open.
