I assume you found the reference on the Kos. Co. web site. There is someone listed who will do those lookups for a nominal fee. I dug out a death certificate that I sent for several years ago. The lady died in 1903 and I did have to contact the state level. The instructions say that deaths prior to 1900 are held at the county. After that, it is State. There are no indexes from 1900-1918. Therefore you must have the county or specific city of death. Exact date is preferable, but they will do a search within a certain timeframe. There is a special form to fill out, which is probably available on the web site. Also must provide identity of the requester such as a photocopied piece of I.D. (driver's license, work ID, etc.) I had to state how I was related to the person and why I wanted the information. Indiana State Dept. of Health Vita Records Section P>O.
