There is a file at the NA for men rejected for service in the CW. My Indiana man did not serve, supposedly for a physical defect. You need to know the Congressional District he lived in at that time. The Indian State Library can give that to you. I need to ck but I think Shelby and area were in the 3rd. ( I will find the letter). I put off sending for mine (back when it was cheap!) and now I am sorry! I think you would put "rejected for service" across the regular form and that would get the job done. You also might see if they have instructions on the NA site, and you can send an e-mail to them, as to what form to use. Hope this helps. Carleen
