The State of Illinois will grant $50 (as of 7/31/97) toward the installation of such a marker if installed on a Veterans grave within Illinois. As the memorial grave marker is a Federal benefit, these are requested by submitting a VA Form 40-1330 (Application for Standard Government Headstone Or Marker, For Installation in a Private or State Veterans' Cemetery). I was informed of the above from the State of Illinois Department of Veterans Affairs. They did not tell me where to get the form. And they did not tell me how to submit for the $50 Illinois benefit. Also, they did not tell me what kind of proof, if any, is needed. Plus, I can't locate my address for the Fed. Dept. of Vets Affairs. Good luck.
