If you do not find another way to obtain this death certificate send: 1--All the names you know that could be connected to LaVerne...full names if at all possible...spouse[s], parents, children, siblings, -- any one else who was close enough to be enlisted to give information 2--All places where you know LaVerne lived or could have lived or that the family lived. 3--All dates that connect to LaVerne, especially death and birth dates. 4--A request for a copy of LaVerne's death certificate. 5--Send your name, address, and ten dollars (to ISGS) to: Death Certificate Chairman Illinois State Genealogy Society PO Box 10195 Springfield, Illinois 62791-0195 You should receive a copy of the certificate within a reasonable time.
