Sharon: Just a note to add on to the info you have received. I just recently sent an e-mail to secryan@ccgate.sos.state.il.us requesting two death certificates. I had the name and date of death and also the Certificate Number. In a reasonably short time I received them in the mail, no charge. I think all you need is the name and date. Kenneth R.
