I have a person in Illinois that looked up some family members for me & sent me a hard copy. On the top of the page it reads ILLINOIS DEATH INDEX 1916-1947.(It's black with the white letters) I believe he copied from microfilm from the Mormon Church LDS. I also know there is an earlier one in the late 1880's to early 1900's & this is on microfilm.(But he didn't have this one.)
