    3. Shelly, Ruth is right. When you order a copy of a Illinois Marriage License. You get the Marriage License and the Marriage Application certificate which is address is the Illinois State Board of Health. It lists Name, Place of Residence, Occupation, Age next Birthday, Place of Birth, Father's name, Mother's Maiden Name, Number of marriage, Married at, and Witnesses, for both the bride and the groom.
