NUMIDENT probably means Number Identification???? I ordered approximately 20 of the Form SS-5 before they went up in price (got one for everyone I could think of). For $27.00 you get a copy of the original form and for $16.00 you will probably just get a computer printout with the information. Didn't the price go up in 2001? Darlene Shawn
