Local funeral homes probably could initiate the action/application but might charge a fee for doing so. This is a picture of the form needed to apply. Note that this is .pdf so it will take some time to load and you will need Acrobat Reader to read it. The form is VA Form 40-1330, Application for standard government headstone or marker for installation in a private or state veterans' cemetery Note that the instruction states that Civil War Request should be made in block 27 of the application and says that it is necessary to submit detailed documentation which supports eligibility .
