Union Banner, Carlyle, IL, Jun 23, 2004, Page 2a/Col 3 Phyllis M. Johnson, 63 Phyllis M. Johnson, 63, of Carlyle died Sunday, June 20th, 2004, at Barnes Hospital in St. Louis. She was born July 2nd, 1941, in Bluffton, Ohio, the daughter of Delmore and Juanita Shumaker Clouse. On Oct. 24th, 1964, in Ohio City, Ohio, she married John W. Johnson, and he survives in Carlyle. Also surviving is her mother, Juanita of Lima, Ohio; a daughter, Diana Hughes of Carlyle; a son, Darin Johnson of Germantown; two grandchildren; and a sister, Carol Linser of Lima, Ohio. She had lived in Carlyle since 1964, and was the cash office manager of the Carlyle Wal-Mart for 22 years. She was a member of the First United Methodist Church and the Order of Eastern Star. She enjoyed reading, crocheting and working on puzzles. A funeral service will be Thursday, June 24th, at 11 a.m. at First United Methodist in Carlyle. Burial will follow in Carlyle Cemetery. Visitation will be today, Wednesday, June 23rd, after 4 p.m. and on Thursday, June 24th, from 9 a.m. until service time at the church. Eastern Star services will be Wednesday evening at the funeral home. Zieren Day Funeral Home in Carlyle is handling the arrangements.
