Hi Kate, Hope this is of some help. It was presented to Alice on April 11, 1919. Inside there is a notation for the year 1939 of Past worthy Matrons. The name given is Agnes O. Wilkinson. Under what I think is Past worthy ???? is a man's name of Stanley N. Fel(or e)??? The page is torn off so I cannot make out the rest of his surname. This was found in an attic on 4339 N. Mozart, Chicago, IL on or about 1965.
