If anybody needs a copy of the "special form" - - IL Dept of Public Health's Application for Search of Birth Record Files of a Deceased Individual - I picked up several this afternoon from IDPH, and they said it was OK for me to scan & email their form. I have it now scanned & converted to .pdf (Adobe Acrobat Reader) format - and IF this List allows attachments, I've attached this "special form" to this email. (it's only 62k) IF the application form cannot be sent out via this List - (and I don't think it's allowed, but it doesn't hurt for me to try~) - then please email me directly at mollx@hotmail.com, with the subject/topic line "special IDPH form".
