    3. Hi Bonnie, It must differ by the timeframe. I had the opposite experience. My application is an exact duplicate of the license itself with none of the choices you mention except age. The bottom of the license itself is filled out with the actual marriage information. Consider yourself lucky that your ancestor was married at that time and not in 1952.
