Dear Sir; I understand Illinois was one of the states that participated in the CIVIL WAR VETERANS QUESTIONAIRE - a special questionaire for all civil war veterans to fill in. I am looking for a copy of this on my ggrandfather, Samuel Washington Brewer, born 1825, entered the service 11 of Feb., 1865 in Co. E of the 66th reg. of Illinois Vols. and was discharged at Camp Butler, IL the 16th day of Jul 1865. He was a resident of Edgar Co., IL or Douglas Co. IL until he died 4 of Jan. 1900. If this is not the office where I should apply for a copy of this questionaire, would you advise me or forward my request on to the correct office.
