Hi Whoever filled out the information on my birth certificate certainly was able to copy the doctor's handwriting so it looks like the doctor did it. I was born at home with the doctor, my father and my grandmother in attendance. It would seem logical to me to fill out a form at the time and the doc do it rather than my father or grandmother Ruth in Vancouver, Wa
