Jean, That's the first I've heard of a form. Lynda gave the url for the Dubuque page and it has a lot of information. So far I haven't had to use DesMoines for any records. I looked up records myself a few years ago when I visited and it is time comsuming so $10.00 is a reasonable fee I think. Thanks so much for your information. Betty
