    3. It's not the same as a death certificate, but you can send for a copy of the Social Security application which has a lot of the same information. Substitute the appropriate information in this form. It's possible to request a copy of SS applications (Form SS-5) of deceased people from the gov't, which gives place of birth, parents names, signature, etc.  Each copy costs US$27.   Here's a form letter generated by the ancestry.com site: ------ < begin form letter > ------- Social Security Administration Office of Earnings Operations FOIA Workgroup 300 N.
