Pat, The last time I sent off for Patriarchal Blessings was about 3 years ago. I didn't need a pedigree chart, just needed to state the relationship on the order form itself. One major word of caution or PATIENCE..... My grandmother's name was ROSAILIA JEPPESEN. Not too hard, right? I knew she had been given a patriarchal blessing, but just couldn't find the card on the microfilm that stated when, where and by whom so that I could order a copy. I just kept searching through the film. One of the very, very last entries to anything that resembled JEPPESEN was this BASILLA JEPSON. AHA!! So, I knew this was grandmother since it listed her mother and father. My patience paid. You can order up to about 10 different ancestors on one form. Makes it easier for you.
