Another thought- I recently heard of a lady in the 19th Century (gggrandmother of a friend) who suffered "post natal depression" but lived on to about sixty in an institution. Could your relative have had a child or a last child at the age of about 38 -otherwise the certifying doctor may have said that she suffered for around 20 years. To give a specific number of years the illness must have been attributable to a specific time or event, something of which the doctor was aware? Did she did at home?
