The name on the certificate is Mrs Annie Gandy. She died on 18 Dec 1910 in Corsicana, Navarro Co, TX. Her husband, W. L. Gandy, signed the certificate. Her father is listed as E.C. Goodnight born IL, mother as Miss Donaldson born MO. Place of burial as Cameron, no funeral home is listed, could be it wasn't known when the form was completed. The family has always "said" she was buried in North Elm Cemetery, Milam Co TX, in a space next to where her husband would be buried. The clue leading me to Navarro Co was the 1910 census for same county, she and her husband, W. L. Gandy were listed as residents.
