A summary of help follows: 1. Try to find the probate court record that sent Lena to Farmington 2. Send a letter to Southeast Missouri Mental Health Center Attn: Health Information Management 1010 W. Columbia Farmington, MO 63640 Send copy of driver's license and death certificate for individual in the institution. 3. Send a letter to ST LOUIS PSYCHIATRIC REHABILITATION CENTER 5300 ARSENAL STREET, ST LOUIS MISSOURI 63139 You will need a copy of your birth certificate, the patient's birth or death certificate, and at least a ballpark figure for treatment dates. This information will allow them to send you documentation pertaining to the individual you have inquired about. If you want the actual treatment records for the patient, you will need a court order. Give this office the information you have on the individual, i.e. birth date, parents, siblings, names person was known by, etc. I did call the Arsenal Street office, and was told I would get hospitalization dates, identification information, probably family information (like next of kin notification, or who signed her in).
