Look on the Soc Security Website <ssa.gov> and follow the links to the application for the SS-5; print it off and fill it out then mail it according to instructions provided, along with the proper fee, you will get a copy of the SS-5 in about 6 weeks...RR Retirement forms are housed in the same format...It helps if you have the SS or RR Number...Sometimes you can find it on the SS Death Index (SSDI) on Rootsweb... All this about creating a Delayed BC for a dead person is useless talk...you CANNOT get one unless the person filed before he died...but you might find a DC in the appropriate State Vital Records, if he died after about 1920 as you indicated...DC have the Parents (and Maiden names of the Deceased when it was a woman), provided the person who gave the information knew the answer...some blocks may say "unknown" indicating they were not knowledgeable of all the facts...
