Greene Street Baltimore, MD  21201 Ask for a Third Party Request for Photocopy - When you get the form send it with the fee (I think its $2.00) You will get a copy of your ancestors application for a SS# - It has a lot of info, like parents name, birth date and death date and place of birth  and death of applicant.
