The number should be at the bottom left hand corner of the page. >> To add to George's comments, this form can be and often is recorded, in Ohio, in the County Recorder's Office. I don't know if this is true in other states. This is done to prevent losing the form since it is used as proof of military service in order to obtain veteran's benefits.
