I have called them at 312-603-6700. They will do a search for 3 years for $7.00 and each additional year is $1.00. I have requested a search for 1930-1940 which cost me $14.00. The address to send to is Cook County Vital Records, P.O. Box 642570, Chicago, IL 60664-2570. I think they will also do birth/death searches for the same fee. Good Luck !! Patty (Lemont, IL)
