When I have our membership directory printed, I make it up using Word, then print it in the size it is supposed to be (about 4.25 x 5 inches), but on letter size paper, leaving much of the page blank. I write the page number in pencil on the blank part of the page. Then I deliver to the printer my printed copies along with a floppy disk containing the Word document. I think they print from the disk, using my printed pages as a guide. They print it with two pages per sheet, then assemble the booklet with a couple of stables at the fold.
