    3. I was just asked a question about obtaining a marker for a Confederate Veteran. My understanding is that this headstone can be obtained through the Dept of Veteran Affairs. Our library had some application forms which I used, and this is the address.... But I don't know how to get the application form itself. Perhaps contact the nearest VA Regional Office, national cemetery or a local veterans' organization. (If anyone has had an actual experience with this, perhaps you can add some helpful details!) VA Form 40-1330 Office of Memorial Programs (403A) Department of Veterans Affairs 810 Vermont Ave. NW Washington, DC 20420 Eligible: Any deceased verteran discharged under conditions other than dishonorable.
