Check the Veterans Administration website for the form, submit the form and wait. You will receive about 3 pounds of documentation of this care and treatment. For my father, they include an oral history interview that had been taken when he was first admitted. It's been some time since I did it, so don't remember all of the details and procedures. I think it helps if you say that you need the medical information for your own medical history. Be sure to include the unit name and number and which war! Good luck!
