In my county/town there is a Veterans Service Officer. He assists families with veterans' services which includes assisting in submitting application for grave markers, if needed. These officers have access to VA records that the general public doesn't have. If there is one in your county (or the county where the marker was placed) he/she may be able to help find out the location, and possibly the information which was submitted on the form.
