Mailmerge forms have not been working for several months, since the Captcha script was broken. Previously, that script was not recognizing a correct entry and would simply return to the Captcha entry form. Something was changed since the Captcha entry is no longer even displayed and attempting to submit produces an error page, but the HelpDesk has told us they do not have a time frame as to when it will be fixed.
