I would appreciate any help or suggestions where to look for info to create a form." I think it would work better to have the email about the survey include a link to the web page with the form. In the HTML form, the action= statement could refer to a script to process the user's input. Depending on how fancy you want to get, it could e-mail the answers to a data-collection person or tabulate and display results. As Barry mentioned, radio buttons are good for multiple choice answers, forcing a choice among alternatives. Also, you could use a pick-list -- the select/option approach. Each of these allows only one answer per question. For questions where you allow more than one answer, use check-boxes. For open-ended questions, use text fields. -rt_/)
