John, I had a couple like this and found that they were scans which had had a separate note pinned to them. So one was of the scan + note and the other with the note folded over so that the full page could be seen.. The notes contained rather confusing comments relating to one of the entries which if I remember I had the transcriber put in as #COMMENT lines . I only uploaded the single file - ie without the ' a ' suffix. Yours may be something else of course.
