I haven't seen any other response, so may I refer you to http://freebmd.rootsweb.com/vol_faq.html#6g which deals with insertions. In some handwritten pages the entries to be inserted appear as you describe, further down (or up) the page, with a little mark by the out-of-sequence entry and a similar mark where it ought to be inserted. The mark is a bit like a x with a + superimposed, in many cases. As you suggest, put the entry in the 'right' place. At least, that is the advice I would give in my syndicate to 'my' transcribers.
