    3. I would see the surname as PETHERICK in the second handwritten entry, and Petherick in the first. However the problem preventing acceptance of your file is you putting in an extra comma in the text in the 'page' column. For the 'page' column you need to put either P106 see M/30 or if WinBMD will allow it "P106, see M/30" if you want to keep the comma. Also I suspect you should use capitals for #COMMENT , as in all the examples in Transcribers' Knowledge Base.
