You need VA For SF-180 (Request for Military Records) which can be obtained from any county veterans service officer in the U.S., many veterans organizations, etc. There is a good explanation of the the process in getting military records at: http://www.va.gov/benefits/pafoia.htm and you can also download a copy of the form at that URL. Be forewarned that most WWII military service records were burned in a collosal fire at the St. Louis Federal Records Repository in 1972. You can probably get proof of service, but most of the details will remain unknown for all time because of that fire.
