Hello, I have just discovered a need to attempt to get the information from a death certificate for a lady who died in Lake County, FL, in 1995. I went to the LDS web site, and they have her in the SSDI listings, as having died in Haines Creek and Leesburg. I checked www.mapquest.com and saw that they are neighboring towns. Betty, Death certs are available from the Office of Vital Statistics, P. O. Box 210, Jacksonville, FL 32231-0042. They cost $5.00 (non-refundable) and it takes 3-4 weeks for processing. You can call 904-359-6900 (recorded msg) to obtain info. Basically it states: send a signed letter stating full name of deceased, sex, death date, city/county, your relationship, your address and your phone number with a check or money order for $5.00.
