    3. You will have to order your death certificate from Jax. The counties do not have them as they are recorded with the state. The address is Vital Statistics, P.O. Box 210, Jacksonville, FL. 32231-0042. Some records date back to 1877, but most date from Jan. 1917. Send check or mo for $5.00 with persons name, dob, and where born, dod, and where, parents names ( if available) and your relationship to the deceased. Takes about 3 weeks. Hope this helps.
